A change log is a dated list of amounts you already chose with a clinician. Add the sleep notes you actually recorded. Add weight. Add symptom notes. Add missed-log facts. Friday then shows a picture. It does not tell a story that the compound caused the change. PepBear will calculate syringe math from numbers you type. It will not propose the next step. Keep the dates. If you only remember a feeling and not the number you drew, leave the row blank instead of guessing a Tuesday amount from a Friday memory.
The amount you intended, the amount you actually drew, the site if you track sites, and whether the row is logged, skipped or unknown.
“Not logged” is not “not taken.” A weekly coach review that collapses those two will lie about the week.
Do not ask it for a starting amount, a weekly increase, or a stack. Those are treatment decisions.
If a paper used 2.4 mg weekly of an approved product, quote that as “used in the cited trial.” Leave it out of your next log unless your clinician already chose it.
Scheduled versus logged, missing check-ins, weight and symptom points, and your reply to last week’s note. Fake sample data lives on the coaches page so nobody’s health record is published.

The product promise is a clear weekly picture for a consented coach. A clinician who is not in that flow still needs the dated records you choose to share outside the app.
